You must mail or fax the following information to the OELMA office after you submit this nomination
form by April 15: (No late Applications will be accepted)
- Letter of nomination which tells the committee the following:
reason for your nomination, nominee’s current and previous professional position(s),
nominee’s educational qualifications, nominee’s professional organization
memberships and offices held, nominee’s activities which warrant this award,
and outstanding achievements of this nominee.
- Letters of support for the Award of Merit, Administrator’s Award, and Scholarship Awards (maximum of three)
- Other supporting documentation (as appropriate)
Scholarship nominees must include:
- college transcript (specify name of program enrolled and expected graduation date)
- financial need
- three letters of recommendation
The Committee and/or Board may elect not to award one or more of the awards during any given year. All
Awards and Scholarships are presented at the Annual Conference unless otherwise noted.
